    A user interface might be described as "retarded" -- but never, so far as I can recall, an actual person.

    I'm normally a big proponent of focussing on intent - I hate when people get worked up by a comment when clearly the intent they are inferring wasn't there.
    But the use of "that's soooo gay" tends to raise my hackles - 9 times out 10 the intent is benign, but the use of the term to describe something bad/substandard/undesirable is coming from a pretty poor and recent history. I kinda put retarded in the same bucket.

    Oh I certainly agree we could attempt to achieve a similar level of public spending, the fact remains that would need to be at higher-percentages of taxation than nations that are producing greater wealth.
    I'm not the guy who says wealth is everything, we have to sacrifice every 3rd child of a Decile 1 school to achieve it. But at least let's recognise that either public or private spending is going to need to be at a lower level than comparative nations. Rather than this dream that we can continue to have top-notch public spending at a 30% tax rate.

    And hell yes we waste spending on historical moral crusades.

    I'll let the authors talk to their reasons for inclusion themselves, but I think this is just the attempt to use extremes to make the point. The "imagery" of someone late in life without family taking a significant portion of the health budget over the young toddler of Porirua (was it?) not getting vaccinated is the attempt to show the extremes of the social outcomes.

    I think you would be hard pressed to actually make numbers of direct family members an actual driver of health spend, and I also don't believe they are trying to do so.
